NYC School Survey – Your feedback is greatly appreciated!
The survey window is from now until March 25. Students will be taking the survey in their advisories.Families can take the survey online via computer or mobile device at 
NYCSchoolSurvey.org in any of the nine DOE supported languages. Paper surveys and prepaid envelopes were also distributed. If you complete the survey online, you do not need to complete the paper version.

The survey will ask you for an access code, which is “f” followed by your child’s nine-digit student ID (OSIS) number. Note that the “f” must be lowercase. You can find your child’s OSIS number on their report card, student iD card, or NYCSA account.
