If your child needs a new Metrocard for any reason (lost, stolen, not working, etc.), please fill out the Metrocard request form and either email it to Ms. Carol at email@example.com or send it in to the main office. It may take a few days for your child to receive a new card. Please be prepared for your child to pay for their fare until they receive a new card. We make every effort to get students a replacement card as soon as possible.
